Embryo transfer is one of the important techniques in IVF which is responsible for giving it a fitting closure or disappointment. All the hard work on stimulation and ICSI will be rendered fruitful only if an embryo transfer is managed with perfection. The said procedure requires the embryologist to load the embryos and the IVF clinician to manage and inject them into the patient. No matter how differently embryo transfer varies across clinics, clinicians, and embryologists, two elements will always remain the same. The differences may arise in the way embryologists load the embryos and the embryo transfer catheter which the clinicians prefer. Another very essential follow up of the embryo transfer technique is checking the contents of the catheter in the form of both inner or outer to determine the retention of the embryos. Apart from these parts, there may be supporting tasks that also can make or break the cycle and render better psychological treatment of the patients. When the patient is taken inside OT for the embryo transfer, calling out her name and keeping two checkpoints is essential considering in ET, the patient may be instantaneously changed for the bladder (if its full or no). The embryologists can have a word with patients regarding the quality of embryos which can be reassuring to them and maintain their spirit. This interaction can be less technical and more basic to make them feel better as the details of the cycle can be explained to them separately with the clinician. Another point can be showing the embryos on the TV screen before loading which does create a positive atmosphere in the OT which is imbibed in pressure and silence. In this particular aspect, the patient can be provided with pictures later on, and whilst loading, the name of the patient on the dish should be ideally displayed.

These small steps are not followed universally but have seen having largely a good impact on the patient especially with respect to the trust factor. The video of the embryo is shown to the patient in the OT just before the transfer but in some clinics, the husbands are seated in a room where they also could see the embryos simultaneously. In the case of filling the bladder, proper coordination between the sisters and the clinicians can help avoid unnecessary chaos as the patients are already stressed and worried about the procedure. In a scenario where we have to handle more than five embryo transfers, managing bladder filling can be a challenging task for the clinic. Any information regarding delays in the transfer or an all freeze prospect should be conveyed in person and frankly should have more humanistic overtones rather than mechanical proofreading. Efforts should be made to not pass information of other patients to the ones getting ready for embryo transfer as it may add more tension to their psyche. In any advent of troubleshooting or embryo retention, the environment or the ambiance should not get into a panic mode. Calmness or silence should be preserved as it will steer the patients with more hope than negative thoughts. In some cases, if the patient feels like lying down on the OT table for more time after the procedure of embryo transfer is completed, they should be granted this privilege if required.

All of the above-mentioned steps can overall make the patient more comfortable and if the cycle fails, they will feel the urge and hope of trying again. Embryo transfer is detrimental in deciding the fate of an IVF cycle and with these little contributions, it can help in not just success rates but ensuring the patients feel better.
